Incident Reporting Form

Please use this form to report any accident, incident or near-miss which took place during any club session or which involved any club member.

Your report will be sent to our club welfare officer and may be shared with other members of our committee. If we require further information, we may contact you via e-mail and/or telephone. Your report may also be forwarded to UK Athletics.
